El brote de nefrolitiasis e insuficiencia renal aguda en ninos, aparecido en China durante 2008, se ha relacionado con la ingestion de formulas infantiles contaminadas con melamina. Estos casos han proporcionado la primera evidencia en humanos de nefrotoxicidad por melamina, previamente descrita solo en animales. Segun el informe del Ministerio de Salud de la Republica Popular de China, 294.000 bebes y ninos pequenos habian sido diagnosticados de litiasis del tracto urinario antes de finales de noviembre de 2008; De ellos, 50.000 ninos habian sido hospitalizados y se habian confirmado seis muertes. En los siguientes meses se han diagnosticado nuevos casos, por lo que se estima que 51.900 ninos han sido afectados. El 82 % eran menores de dos anos de edad; No han aparecido casos en adultos. En la Union Europea no se han diagnosticado casos porque la importacion de productos lacteos procedentes de China esta prohibida por ley desde 2002. Los calculos son radiotransparentes y no se ven en las radiografias simples, es necesaria la ecografia. Debido a la falta de sintomas en la mayoria de los ninos con litiasis, la evaluacion ultrasonografica es la optima para la deteccion de estos calculos renales. Teniendo en cuenta que es demasiado pronto para saber que consecuencias tendra a largo plazo la nefrotoxicidad por melamina encontrada ahora, el panel de expertos patrocinado por la FAO, la OMS y el Ministerio de Salud de Canada recomienda hacer seguimiento a largo plazo de los ninos que han presentado intoxicacion por melamina
