Abstract

60 歳,女性.失語症および右半身麻痺が出現し,近医より脳梗塞の診断にて当院紹介となった.当院搬入時 NIHSS スコア 30 点.前医撮影の頭部 magnetic resonance angiography(MRA)にて左内頸動脈が閉塞しており,MRI の DWI にて左基底核および左放線冠に高信号病変を認めた.Alberta stroke programme early CT score(ASPECT)-DWI 7 点.rt-PA 静注療法および血管内治療により再開通が得られ,治療 18 日後に mRS 1 にて自宅退院となった.治療 1 カ月後に右半身の違和感を自覚し当院を再受診した.頭部 MRI の DWI および fluid attenuated inversion recovery(FLAIR)にて治療側の左大脳白質に新たな高信号病変を認めた.症状の悪化がないため経過観察をしたところ,一部病変は残存したが,半年後には白質病変の改善傾向を認めた.急性期再開通療法後に遅発性白質病変が出現した症例の報告は非常に少ない.原因として遅発性低酸素白質脳症および親水性コーティングによる肉芽腫性病変が指摘されているが,原因の解明には症例の蓄積が必要である.
