Pharmacological Support of Neurorehabilitation of Patients after Ischemic Stroke from the Position of Evidence-Based Medicine

Высокая медицинская и социально-экономическая значимость проблемы ишемического инсульта (ИИ) определяется существенным влиянием на такие демографические показатели, как заболеваемость, инвалидность и смертность. Одной из наиболее важных причин утраты трудоспособности после инсульта является нарушение функции верхней конечности, которое встречается практически у 80% пациентов, причем у 40% - сохраняется пожизненно. Это приводит к ухудшению мобильности, ограничению независимости в повседневной жизни, уменьшает шансы на возврат к профессиональной деятельности и, как следствие, снижает качество жизни пациента. Таким образом, восстановление двигательной функции верхней конечности является одной из приоритетных задач у пациентов, перенесших инсульт. Центральным звеном функционального восстановления после инсульта является нейропластичность. Церебролизин - это препарат, обладающий мультимодальным механизмом действия, влияющий на эндогенную защитную активность головного мозга посредством плейотропных терапевтических эффектов путем одновременной модуляции нескольких компонентов патологического каскада. В подострой стадии инсульта комбинированная терапия церебролизином совместно со стандартизированной программой реабилитации обладает потенциальным синергическим действием. Недавно проведенные исследования показали, что применение церебролизина способно стимулировать эндогенную нейропластичность и, в сочетании с ранней двигательной реабилитацией, способствует восстановлению утраченных двигательных функций, атакже улучшению общего исхода у данной категории пациентов. На основании доказательной базы, полученной в клинических исследованиях, церебролизин был включен в Австрийские, Немецкие и Канадские руководства по реабилитации после инсульта. В 2021 году церебролизин был рекомендован руководством по фармакологической поддержке в ранней двигательной реабилитации после острого ИИ, разработанным представителями Европейской академии неврологии и Европейской федерации обществ нейрореабилитации. The high medical and socio-economic significance of the problem of ischemic stroke is determined by the significant impact on such demographic indicators as morbidity, disability and mortality. One of the most important causes of disability after a stroke is the upper limb dysfunction, which occurs in almost 80% of patients and in 40% of patients, it persists for life. This leads to mobility decline, limited independence in everyday life, reduces the chances of returning to professional activity and, as a result, contributes to decrease of the quality of life. Thus, the recovery of the upper limb motor function is one of the priority tasks in patients after stroke. Neural plasticity represents the core of functional recovery after stroke. Cerebrolysin has a pharmacologically multimodal mechanism of action, influencing the brain’s endogenous defense activity through pleiotropic therapeutic effects by simultaneously modulating several components of the pathological cascade in stroke. Combining Cerebrolysin treatment with the standardized rehabilitation program may have a potential synergistic effect in the subacute stage of stroke. Recent clinical studies showed that Cerebrolysin can stimulate endogenous neuroplasticity and in combination with early motor rehabilitation has a beneficial effect on motor function and global outcome in this category of patients. Cerebrolysin, based on the evidence of the clinical studies, has been recommended in the Austrian, German and Canadian guidelines for rehabilitation after stroke. In 2021, Cerebrolysin was included in the evidence-based guideline on pharmacological support in early motor rehabilitation after acute ischemic stroke developed jointly by the representatives of the European Academy of Neurology and the European Federation of Neurorehabilitation Societies.
