症例は68歳の男性で,Cronkhite-Canada症候群(以下,CCSと略記)の診断にてステロイドを中心とした内科的治療を継続するも無効で3.5か月が経過していた.ポリポーシスが原因の終末回腸浮腫による通過障害にて成分栄養の経口投与で栄養管理され,proton pump inhibitor(以下,PPIと略記)は経口投与されていた.右側腹部痛の急激な増悪を認め,上部消化管穿孔の診断のもと緊急手術を施行した.十二指腸球部前壁の穿孔部分への大網充填に加え,通過障害を呈していた回盲部を切除し,回腸人工肛門造設術を併施した.術後はステロイド,抗TNFα抗体の投与にてCCS症状が改善した,またPPIも術直後の静脈投与から内服に変更したが十二指腸潰瘍の再発は認めなかった.内科的治療抵抗性のCCS症例では主病変の外科的切除を行うことも考慮する必要があると考えられた.
