Abstract
【目的】当院における人工股関節全置換術・人工骨頭置換術後に生じたステム周囲骨折の治療成績を検討した．【対象・方法】対象は，7例7股で男性2例，女性5例，平均年齢66歳，平均観察期間は60カ月であった．Vancouver分類Type Aは1例でケーブルグリップを使用し，Type B1は3例でワイヤリングのみが1例，Mennen plate®併用が1例，Locking compression plate（以下LCP）併用を1例に行った．Type B2の2例はステムの再置換を行い，Type Cの1例はLCPとワイヤリングを用いて骨接合を行った．【結果】7例中6例は平均6カ月（3～12カ月）で骨癒合を得た．Mennen plate®使用の1例で偽関節となり，再骨接合術後に感染し，経過中に死亡した．再置換を行った1例で術後感染し，ステム抜去し経過観察中である．残りの5例は受傷前と同レベルの日常生活へ復帰した．【考察】Vancouver分類は治療方針決定に有用であった．LCPはステム周囲骨折に対して強固な内固定として有用と思われた．
