Abstract
Bronchiolitis obliterans syndrome (BOS) is the leading cause of graft loss after lung transplantation.Recent data from an orthotopic rat lung transplant model strongly implicate CD4+ T cells specific for peptides derived from col (V), an important constituent of the lung ECM, in the pathogenesis of airway destruction/remodeling both in allografts and isografts.To test the hypothesis that autoimmunity to col (V) predisposes to BOS in clinical lung transplantation we sampled PBMC 0.5-3 yrs.after lung transplants in n=31 pts, including 15 single and 16 bilateral Control PBMC were from normal donors (n=5) or pre-transplant pt samples(n=4).PBMC were tested using the transvivo DTH assay in a SCID mouse footpad for swelling reactions to col (V), or to control col (II) not expressed in the lower airway.BAL samples were obtained from a subset of 16 pts; the IgG2 fraction of the BAL fluid was purified by affinity chromatography and tested in Western blot vs. purified col (V), or col (II) .RESULTS: Control and pre-tx values for anti-col (V) DTH ranged from 0-20 over PBS background; none of the control BAL samples contained anti-col(V) IgG antibodies.As shown in Table 1, there was a significant (p<.05) association of development of a positive anti-col V DTH response and onset of BOS .PBMC from 6/ 6 pts with BOS development in the first 3 years caused significant (≥25 ) swelling of a SCID mouse footpad in response to col V either before or at the time of BOS diagnosis; 3 have died, 2 have severe BOS(grade 3) and 1 has mild BOS (grade 1).All 11 of the col (V) DTH-negative pts are currently in the non-BOS category, which also includes 14 col(V) DTH+ responders.Anti-col (V) DTH was strongly associated (p<.02) with local production of anti-col(V) IgG2 Abs in the lung(s), suggesting a common Th-dependent process.CONCLUSION: There is an association of anti-col (V) autoimmunity with BOS in lung transplant patients, suggesting that tolerogenesis strategies using col(V) may be of value in preventing BOS.Association of anti-col V DTH (≥25 over background) with BOS Group DTH Negative DTH Positive BOS 0 6 non-BOS 11 14 p<.05 by Pearson's test; values=no. of patients Anti-col V IgG in BAL fluid correlates with a positive anti-col V DTH Group: DTH negative DTH positive IgG2 anti-(V)+ 0 7 Ab negative 5 4 p<.02 by Pearson's test; values=no. of patients
