Abstract

Infection following total knee arthroplasty is a devasating complication. Prophylactic systemic antibiotics are well accepted at reducing the rate of infection. Acrylic bone cement is commonly used for component fixation in primary arthroplasty and good evidence exists for the addition of low dose antibiotics to bone cement. Decades of experience with its use in European and Scandanvian countries accounts for its excellent safety profile and efficacy. Despite this, there is still controversy regarding its use in North America. This differs from the use of high dose antibiotic laden bone cement in the treatment of established joint infections which is universally accepted. Theoretical concerns regarding; alteration in mechanical strength, potential for the development of drug resistant microbes, systemic toxicity and allergic reactions, remain unsupported. The increased associated cost is offset by a reduction in the infection rate. Routine use of low dose antibiotic bone cement in primary knee arthroplasty is advocated.
