Abstract
A previously well 13-year-old girl presented to hospital with a right focal seizure involving the hand, mouth and eye, which progressed to a secondary, generalized, tonic-clonic seizure lasting 10 min. Subsequent to this, she suffered postictal confusion and a generalized headache. The patient had no other localized or constitutional symptoms. Her medical history included one hospital admission at one year of age for cellulitis. Born to Afghani and Pakistani parents, she was raised in New Delhi, India, and immigrated to Canada one year before her presentation. Her immunizations were updated on arrival in Canada. There were no known tuberculosis contacts or history of tuberculosis in her family. She took no medications, and had taken no over-the-counter or illicit drugs. She was seen initially in a community hospital, where her seizure was successfully controlled with a loading dose of intravenous phenytoin, and a computed tomography (CT) scan of her head revealed an enhancing lesion in the left frontal cortex. Accordingly, she was transferred to a tertiary care centre for neurosurgical assessment with a provisional diagnosis of central nervous system glioma. A further history revealed no risk factors for HIV, and no family history of vasculitis or thrombosis. Her physical examination and all bloodwork, including complete blood count, electrolytes and toxicology screen, were normal. Further investigations revealed the diagnosis.
