Abstract
Purpose: Despite its benefits in preventing cardiovascular diseases in the general population, the aerobic exercise, even of moderate load, is not recommended in patients with the Marfan syndrome (MF).We have previously demonstrated in an animal experimental model the presence of indirect signs of pressure overload in the myocardium of individuals with this syndrome that could be in turn improved with moderate exercise.Our objective was to evaluate the impact of exercise on aortic dilatation and myocardial involvement in an experimental murine model of MF.Methods: We studied 20 wild-type (WT) and 20 Marfan (MF) FBN1C1039G/þ mice (50% female) divided into 4 groups: WT sedentary (10), WT athletes (10), MF sedentary (10) and MF athletes (10).Both WT-and MF-athlete groups underwent 5 months of aerobic training (1 hour/day of running on a treadmill at 20cm/s and 30 slope).Echocardiography was performed at baseline (4 months old) and after 5 months of training and included measurements of aortic root and left atrium size, anterior and posterior wall (AW, PW) thickness, left ventricular diastolic and systolic diameters (LVDD, LVSD), ejection fraction (LVEF) and Tissue Doppler images (TD).The presence of basal septal post-systolic thickening (PST), a marker of myocardial pressure overload, was assessed with both M-mode and TD.Blood pressure was measured at baseline and at the end of the experimental protocol.RESULTS: At baseline, Marfan mice presented with a larger aortic root diameter (1,9 vs 1,48 [mm]; MF vs WT), LV dilation (LVDD, LVSD) but no differences in systolic function (LVEF), thicker ventricular walls (AW, PW) with increased presence of PST (84%) and left atrium dilation.We found no differences in blood pressure between Marfan and WT mice.After 5 months of aerobic training, (i) systolic blood pressure was slightly lower in WT athletes than in MF athletes, (ii) aortic root was significantly smaller in MF athletes than in MF sedentary, (iii) subclinical hemodynamic overload (PST) was blunted in MF athletes (40%) in comparison to their sedentary littermates (71%), which in turn was associated with a decreased LV diastolic dilation and wall thickness in trained MF.CONCLUSIONS: The moderate exercise activity improves cardiovascular echocardiographic parameters in an experimental murine model of MF, suggesting that this type of exercise could even be recommended to human patients.
