Abstract
Einleitung: Seit der Einführung der Sentinellymphknotenbiopsie (SLNB) zum axillären Staging bei Mammakarzinompatientinnen, hat sich diese Technik in den meisten Brustzentren etabliert.Methoden: Von März 1998 bis März 2002 wurde an 500 Patientinnen mit invasivem Mammakarzinom die SLNB durchgeführt.In der Lernphase wurde in derselben Narkose nach erfolgter SLNB die Axilladissektion (ALND) durchgeführt.In der zweiten Phase wurde bei Patientinnen mit negativem Sentinellymphknoten (SLN) auf die Axilladissektion verzichtet.In einer dritten Phase wird die SLNB auch bei Patientinnen mit multizentrischen Karzinomen und bei Patientinnen nach neoadjuvanter Chemotherapie evaluiert.Zur Auffindung der SLNs wird die kombinierte Methode mit Injektion von blauem Farbstoff und mit Injektion von radioaktivem Tracer angewandt.Ergebnisse: 500 SLNBs wurden durchgeführt.Die Gesamtauffindungsrate betrug 86,2%.Nach Bereinigung der Daten durch Weglassen der Patientinnen mit multizentrischen Karzinomen und der Patientinnen nach neoadjuvanter Chemotherapie, betrug die Auffindungsrate 94,5%.Insgesamt war der SLN in 41,3% der Patientinnen positiv 58,7% der Patientinnen negativ.In 60,1% der SLN-positiven Patientinnen war der SLN der einzige positive Lymphknoten und die Lymphknoten der Restaxilla waren negativ.Von den im Gefrierschnitt und in der H&E Färbung negativen SLNs wurden 7,3% durch die zusätzliche Immunhistochemie (IHC) von negativ auf positiv konvertiert.Zusammenfassung: Die SLNB ist eine exakte Methode zur Bestimmung des axillären Status bei Mammakarzinompatientinnen. Unter bestimmten Voraussetzungen wird an manchen Zentren bei negativem SLN auf die Axilladissektion verzichtet.Die Datenlage zur Anwendung der SLNB bei multizentrischen Karzinomen und nach neoadjuvanter Chemotherapie ist kontrovers und bedarf noch weiterer Studien.
