Abstract
A 1-month-old previously well term infant presented to hospital with a 1-week history of pallor, 2 days of non-bloody, non-billious emesis, and tactile fevers. The baby was born by repeat Caesarian section following a healthy pregnancy. Her mother received routine antenatal care and had reassuring serologies and ultrasounds. The infant’s birth weight was 3 kg and there was no resuscitation needed at birth. She was exclusively breastfed and growing well. Review of systems and family history was noncontributory. On examination, she appeared fatigued and pale, had a fever of 39°C, and had mild tachypnea. The rest of her examination was normal apart from mild abdominal distension. Initial investigations showed anaemia (78 g/L); leukocytosis (25.7×109/L) with elevated neutrophils, and thrombocytopenia (26×109/L). C reactive protein was elevated at 213 mg/L as were the ALT (192 U/L) and AST (449 U/L). A lumbar puncture was unsuccessful. Blood smear showed no hemolysis but had some cells concerning for blasts. She was admitted to hospital for possible sepsis and received empiric cefotaxime, vancomycin, and acyclovir. Despite this, her transaminitis and thrombocytopenia failed to improve so she was transferred to a tertiary paediatric centre for further assessment.
