Abstract
A healthy 13-year-old Chinese girl presented with a 9-month history of enlarging, asymptomatic, linear plaques on her right cheek. The patient reported no fever, weight loss, photosensitivity, oral ulceration, alopecia, arthralgias, or muscle weakness. She had no previous history of preceding trauma around the lesions or frequent exposure to sunlight. She had no family history of collagen vascular diseases. Physical examination showed multiple well-defined, noninfiltrated, scaly, erythematous plaques in a Blaschkoid distribution on her right cheek (Figures 1 and 2). Skin atrophy with telangiectasia was noted. The rest of the examination was unremarkable. Laboratory tests such as urinalysis, complete blood count, C-reactive protein, serum complement 3 and 4, liver function tests, and renal function tests were normal. Antinuclear antibodies (ANA), extractable nuclear antigen antibodies (ENA), and perinuclear antineutrophil cytoplasmic antibodies (P-ANCA) were negative. A skin biopsy of the lesion showed epidermal atrophy, loss of rete ridges, and interface changes with focal basal cell vacuolization and apoptotic keratinocytes; perifollicular and perivascular lymphocytic infiltrates; dermal mucin deposition; and no thickening of the basement membrane.
