Introdução: A insuficiência cardíaca (IC), síndrome clínica complexa caracterizada pela incapacidade do coração de atender às demandas metabólicas tissulares em pressões de enchimento adequadas, afeta entre 1% e 2% da população adulta mundial. Apesar da disponibilidade de diversas terapias, a IC ainda está relacionada a altas taxas de morbimortalidade, destacando-se, portanto, a necessidade de novas estratégias terapêuticas. Nesse contexto, estudos com os inibidores SGTL2, inicialmente utilizados como hipoglicemiantes, surgem como uma nova possibilidade. Objetivos: Discutir os possíveis benefícios e mecanismos de ação dos inibidores SGLT2, além de analisar seu uso no tratamento da IC. Metodologia: Foram realizadas buscas na base de dados PubMed, de sete artigos no idioma inglês, datados de 2017 a 2020, utilizando os descritores “Sodium-Glucose Transporter 2 Inhibitors”, “Heart Failure” e “Drug Therapy”. Discussão: Diversos estudos mostraram o efeito benéfico dos inibidores SGLT2 em pacientes com IC. O ensaio DOPA-HF demonstrou a aplicabilidade de dapagliflozina no tratamento de ICFEr em pacientes sem diabetes; o estudo EMPEROR-reduced identificou uma redução na mortalidade e na hospitalização por IC nos pacientes com ICFEr tratados com empagliflozina. Sugere-se que esses fármacos apresentam como efeitos cardioprotetores a inibição da remodelação cardíaca, o aumento da natriurese e da eritropoiese e a redução da pressão glomerular. Conclusão: Os inibidores SGLT2 são candidatos promissores para o tratamento de pacientes diabéticos e não diabéticos diagnosticados com IC. São necessários novos estudos envolvendo diferentes populações, a fim de corroborar os benefícios do uso dessa classe de medicamentos para estes pacientes.
