Abstract
Cigarette smoking is the leading global cause of chronic obstructive pulmonary disease (COPD). Worldwide, more than 1.3 billion persons smoke and 384 million persons have COPD.1 COPD is clinically diagnosed on the basis of persistent airflow limitation as measured by spirometry in persons with a history of smoking who have frequent respiratory symptoms.2 One major limitation of spirometry is that the values it measures mostly reflect pathologic processes in the large airways. However, in COPD, the disease begins in the small airways (i.e., airways <2 mm in diameter); these airways are inflamed, narrowed, and then destroyed in response to long-term . . .
