Pressor Response To Blood-Flow Restriction Exercise: Influence Of Muscle Mass And Central Blood Volume Reduction

Abstract
Blood-flow restriction (BFR) exercise has become popular in sport and rehabilitation owing to its efficacy to induce training effects with low external loads. During BFR exercise, reductions in venous outflow from the active limbs may limit the exercise pressor effect due to competing factors (metaboreflex vs. reduced venous return). It is unclear if the degree of blood trapping, dictated by the active muscle mass, influences this pressor response. PURPOSE: Determine whether reductions in venous return may impact the pressor response during resistance (RE) and BFR RE. METHODS: Participants (n = 14) performed RE and BFR using single- (SL) and double-leg (DL) leg press to failure. Continuous blood pressure was recorded with photoplethysmography and expressed as a change from baseline. A subset of (n = 7) performed isometric handgrip (HG) and post-exercise circulatory occlusion (PECO) during lower body negative pressure (LBNP) at -20 and -40 mmHg to reduce venous return. RESULTS: Compared to SL conditions, exercising systolic blood pressure (SBP) was further elevated with LL-BFRDL (55 ± 18 vs. LL-BFRSL = 44 ± 17 and LL-RESL = 40 ± 12 mmHg, both p ≤ 0.05), but not LL-REDL (46 ± 18 mmHg, p = 0.8). Diastolic blood pressure (DBP) during exercise similarly increased with BFRDL compared to all RE (38 ± 9 vs. RESL = 25 ± 7 and REDL = 28 ± 9 mmHg, both p ≤ 0.01), whereas BFRSL (32 ± 10 mmHg) differed only from RESL (p = 0.02). In the rest periods, similar pressor responses were seen for SL and DL BFR (both SBP: 28 mmHg; DBP: 11 mmHg) and DBP responses were greater than all RE (RESL = 3 ± 6 mmHg and REDL = -3 ± 8 mmHg, all p ≤ 0.05). SBP increased from baseline with HG and PECO; however, the increase did not differ across staged LBNP (p > 0.3), despite HR increasing during PECO (3-4 bpm/stage, p = 0.05, HG n.s). DBP showed increases with greater LBNP during HG (LBNP0 = 20 ± 15, LBNP20 = 27 ± 10, LBNP40 = 30 ± 11 mmHg, p = 0.03). CONCLUSION: The SBP response was moderately elevated with BFRDL compared to BFRSL exercise, but attenuated in the rest periods, suggesting an effect of muscle mass during exercise only. Graded reductions in venous return resulted in an increase in DBP during an upper body stressor challenge, yet no change in SBP responses despite an apparent compensatory attempt to increase cardiac output with further cardiovascular stress.
