S34 Evolving Trends and Concentrations in Inflammatory Bowel Disease Research 2000-2020

Abstract
Background: Inflammatory bowel disease (IBD) is a chronic inflammatory disease with complex pathophysiology and treatment. There has been an accelerating rise in both incidence and prevalence since the second half of the 20th century. Addressing this increasing burden requires a strong evidence base on IBD diagnosis, management, natural history and complications. The objective of this study is to assess the status of IBD research worldwide and highlight potential gap areas. Methods: We searched the Web of Science core collection database for peer-reviewed publications related to IBD. Search results were limited to the period between years 2000 and 2020. The publications were divided by the country of affiliation of the corresponding author, funding status and research topics. The number of publications was normalized to the IBD national prevalence and incidence for each country using data from Institute for Health Metrics and Evaluation. Additionally, the articles were categorized into research topics via the use of predetermined terms based on title, key-terms and abstract words; these categories are not mutually exclusive, for example a publication comparing medical therapy to surgery will be present in both categories. Results: A total of 25,640 articles were retrieved from the search. Trends show a steady rise in the number of publications with an almost 5 times increase from 2000 to 2020. Subdivision of the timelines by countries shows the highest rise for China and the USA. 88% of the publications originated from just 20 countries with USA, China, and Japan being the top 3. The collaboration for these twenty most prolific countries with other countries was limited to only 19% of the articles originating from a multinational collaboration, with Canada (33%), Belgium (30%) and Sweden (29%) having the most collaborations. When standardizing the number of publications by the IBD incidence and prevalence, the most prolific countries respectively would be China, USA, and India. The most common research topics included FDA-approved medications (n = 4526, 17.7%), oncology in IBD (n = 3622, 14.1%) and surgery (n = 3074, 12.0%). In contrast, there were fewer studies of quality of life (n = 864, 3.4%), microbiome (n = 786, 3.1%), health maintenance (n = 692, 2.7%) and epidemiology (n=452, 1.8%). Furthermore, a differential in the funding distribution exists among the topics with 14.9% of the funded articles addressing FDA-approved medications and 12.5% for genetics while less for quality of life (3.2%), epidemiology (2.1%) and health maintenance (2.0%). Conclusion(s): Despite an immense rise in the number of IBD related publications from 2000 to 2020, discrepancies do exist in the distribution of publications across the globe and across the research topics. Several clinically unmet needs for IBD patients were reflected in the discrepancies in the research topics.
