Abstract
A 27-year-old woman presented to the emergency department with a 5-month history of abdominal pain and a 7-kg weight loss. She had no medical or surgical history and did not take medications. She immigrated to Canada from the Philippines 11 months before symptom onset. The month before presentation, she experienced the onset of night sweats and fatigue. She also reported shortness of breath on exertion. On presentation, heart rate was 111 beats per minute (regular); she was afebrile. Her body mass index was 16.8 kg/m2. Head and neck examination revealed supraclavicular lymph node enlargement and scarring of the right side of her neck from which she described spontaneous, intermittent drainage (Figure 1). Auscultation of her chest revealed reduced breath sounds in the right lung base. On abdominal examination, there was guarding on palpation of the left lower and right upper quadrants. Pelvic and rectovaginal examinations revealed an anterior mass and a palpable bulge in the cul-de-sac. The cervix was noted to feel normal on bimanual examination.
