La enfermedad de Alzheimer (EA) es un proceso degenerativo del sistema nervioso que junto a los cambios histopatológicos clásicamente conocidos de formación de placas seniles y ovillos neurofibrilares se acompaña de una variable pérdida de neuronas. Se ha postulado que esta muerte neuronal puede ocurrir por un mecanismo de apoptosis. Las células en apoptosis pueden identificarse añadiendo mediante la enzima terminal-deoxinucleotidil-transferasa (Tdt) nucleótidos marcados a los terminales 3'-OH que se liberan al fragmentarse el ADN a nivel internucleosomal. Con esta técnica se han identificado neuronas en apoptosis en cerebros de 5 pacientes con EA con más frecuencia que en controles de edades correlativas. También se han estudiado por técnicas de inmunohistoquímica la expresión de la proteína antiapoptótica bcl-2 y no se ha encontrado inmunorreactividad en neuronas pero sí un aumento de la misma en astrocitos en cerebros de pacientes con EA con respecto a los controles. La elevada expresión de bcl-2 en células gliales en la EA contribuiría a su propia supervivencia y podría a su vez influir en la pérdida neuronal.
