Abstract
Purpose: Parkinson’s disease is a neurodegenerative disorder of dopaminergic cells in basal ganglion. The standard rehabilitation models like non-invasive cranial stimulation aims to optimizing motor and cognitive recovery and minimize functional limitations in individual with PD. The aim is to determine the effect of NIS in motor and cognitive domains in an individual with Parkinson’s disease. Participants: Single group of 4 participants within the age group of 45-75 years were prediagnosed with Parkinson’s disease with motor impairment (Hoehn and Yahr stage I-III) and cognitive impairment were included. Methods: The present study was the part of Randomized control trial (RCT) registered in CTRI no:26/55/IEC/PUP/2022. Institutional approval was taken by Punjabi University Patiala. In this, patients were intervened with rTMS over DLPFC for the period of 20 minutes for 15 days combined with conventional physiotherapy. Outcome assessment was done using MDS- UPDRS - Version-III, MOCA, DRS and PD-QOL. Post-intervention Assessment was done at Day 7th and Day15th. Results: Improvement was observed in all the outcome measures i.e Montreal Cognitive Assessment (MOCA), Movement Disorder Society Sponsored Revision of Unified Parkinson Disease(MD-UPDRS), Disability Rating Scale (DRS) across the span of 2 weeks in which for MD-UPDRS value of p=.002,(t=9.899), MOCA (p=.069)(t=2.777), DRS(p= .092)(t=2.449).rTMS emphasized in the present cases is an insight for enhancing motor recovery and improvement in cognitive functioning by stimulating targeting area. Conclusion: The present results demonstrated the beneficial effect of Repetitive Transcranial magnetic stimulation in DLPFC on motor functions of UL & LL and cognitive function of the patients. Implications: Non-Invasive Stimulation enhancing motor recovery and improvement in cognitive functioning by stimulating targeting area.
