Psychological Features that Induce Stress Perception in Patients with Psoriasis and Atopic Dermatitis

Введение. Стресс считается одной из причин обострения псориаза и атопического дерматита. Цель. Изучить проявления алекситимии, стрессоустойчивость, копинг-стратегии, уровень соматизации, механизмы психологической защиты и эмоциональный интеллект у пациентов с псориазом и атопическим дерматитом. Материалы и методы. Обследовано 67 пациентов с псориазом, 59 – с атопическим дерматитом, 50 – с невротическими, связанными со стрессом и соматоформными расстройствами, без кожной патологии (далее – невротические расстройства) и 65 практически здоровых людей. Клинические методы: клинический осмотр и интервьюирование. Психодиагностические методы: алекситимия изучалась с помощью 20-пунктовой Торонтской шкалы алекситимии; стрессоустойчивость оценивалась методом Холмса – Раге; копинг-стратегии оценивались с помощью опросника «Способы совладающего поведения» по Лазарусу; психологические защиты изучались методом Плутчика – Келлермана – Конте; эмоциональный интеллект оценивался опросником Дж. Маера, П. Сэловея и Д. Карузо. Результаты. Пациенты с псориазом и пациенты с невротическими расстройствами демонстрируют затруднения в эмоциональной осведомленности, способности контролировать эмоции и преодолевать стресс эффективным методом по сравнению с практически здоровыми лицами. У пациентов с псориазом сильнее выражено отсутствие способности принимать существующие проблемы, чем у пациентов с невротическими расстройствами. Уровень эмоциональной осведомленности выше у пациентов с атопическим дерматитом, чем у пациентов с невротическими расстройствами. Уровень соматизации у пациентов с атопическим дерматитом ниже, чем у пациентов с невротическими расстройствами. Для пациентов с атопическим дерматитом изученные характеристики психологических особенностей личности, влияющих на восприятие стресса, не проявили статистически значимых различий по сравнению с пациентами с псориазом и практически здоровыми лицами. Низкая стрессоустойчивость при псориазе ассоциирована с нарушением эмоциональной осведомленности, примитивными механизмами психологической защиты, соматизацией и дезадаптивными копинг-стратегиями, а при атопическом дерматите – с примитивными механизмами психологической защиты и дезадаптивными копинг-стратегиями. Заключение. При обследовании пациентов с псориазом и атопическим дерматитом следует обратить внимание на их личностные психологические особенности, которые, возможно, могут препятствовать успешному преодолению стресса и быть причиной обострения данных патологий. Introduction. Stress is considered to be one of the reasons of psoriasis and atopic dermatitis. Purpose. To study alexithymia, level of stress resistance, coping strategies, somatization, psychological defense mechanisms and emotional intelligence in patients with psoriasis and atopic dermatitis. Materials and methods. 67 patients with psoriasis, 59 – with atopic dermatitis, 50 – with neurotic disorders and 65 healthy controls were enrolled in the study. Clinical methods: clinical examination and medical interview. Psychodiagnostics: alexithymia was assessed by The 20-item Toronto Alexithymia Scale; stress contribution to illness was assessed by The Holmes and Rage Stress Inventory; coping-strategies were assessed by The Ways of Coping Checklist; somatization was assessed by SOMS-2; psychological defense mechanisms were assessed by Life Style Index; emotional intelligence was assessed by MSCEIT 2.0. Results. Patients with psoriasis and patients with neurotic disorders show difficulty in emotional awareness, ability to control emotions and cope with stress in an effective way compared to the healthy controls. The ability to accept existing problems lower in patients with psoriasis than in patients with neurotic disorders. The level of emotional awareness is higher in patients with atopic dermatitis than in patients with neurotic disorders. The level of somatization in patients with atopic dermatitis is lower than in patients with neurotic disorders. Studied psychological features that induce stress perception in patients with atopic dermatitis did not show statistically significant differences compared to those in patients with psoriasis and healthy individuals. Low emotional awareness, maladaptive coping strategies, primitive psychological defense mechanisms and somatization are increasing chances of low stress resistance in patients with psoriasis. Maladaptive coping strategies and primitive psychological defense mechanisms are increasing chances of low stress resistance in patients with atopic dermatitis. Conclusions. Some psychological features can prevent from successful stress management and cause relapses in patients with psoriasis and atopic dermatitis.
