Au sein des institutions médicales occidentales, la relation de soin est aujourd’hui caractérisée par l’asymétrie, car les soigné·e·s dépendent des soignant·e·s. Le rétablissement, voire la survie des premier·ère·s – dont les connaissances médicales et l’autonomie sont limitées – repose sur la compétence des deuxièmes1. Pour Agata Zielinski, cette disparité entre les deux membres de la relation est manifeste au premier coup d’œil : « L’un est en général debout, l’autre allongé ou assis ; l’un vêtu d’une blouse blanche au titre de sa fonction, l’autre en pyjama ou dénudé, signe de sa vulnérabilité2 ». Bien entendu, l’inégalité de pouvoir, dont Zielinski donne une illustration éloquente, n’engendre pas forcément un rapport de domination3, sans compter qu’il existe désormais des mécanismes pour limiter les risques de dérives, comme l’obligation d’informer et d’obtenir le consentement éclairé du patient4. Si les dernières décennies ont été le théâtre d’une remise en question du paternalisme médical, il reste encore beaucoup de travail à faire pour parvenir à un changement de paradigme, surtout en ce qui concerne les rapports entre le personnel médical et les populations marginalisées5. Comment éviter que la relation de soin ne donne lieu à des abus de pouvoir ? La littérature peut-elle contribuer à l’établissement d’un rapport plus équilibré et si oui, comment ?
