To carry out a radial forearm flap, the radial artery is usually harvested, incurring severe donor site morbidity. Advances in anatomical knowledge discovered constant radial artery perforating vessels, enabling the subdivision of the flap into smaller components suitable for a wide range of differently shaped recipient sites, with marked reduction of downsides.Eight pedicled or free shape-modified radial forearm flaps were used to reconstruct upper extremity defects between 2014 and 2018. Surgical technique and prognosis were examined. Skin texture and scar quality were assessed on the Vancouver Scar Scale while function and symptoms on the Disabilities of the Arm, Shoulder and Hand score.At a mean follow-up of 39 months, no cases of flap necrosis, impaired hand circulation or cold intolerance were found.The shape-modified radial forearm flap is not a new technique, but is poorly known by hand surgeons; in contrast, our experience showed it to be reliable, with acceptable functional and esthetic outcomes in selected cases.Le sacrifice de l'artère radiale est l'inconvénient majeur du lambeau antébrachial radial. Les connaissances anatomiques récentes ont mis en évidence la présence constante de vaisseaux perforants de l'artère radiale qui permettent la subdivision de ce lambeau en composants de taille inférieure, compatibles avec plusieurs sites receveurs de formes variées.Huit lambeaux antébrachiaux radiaux à forme modifiée (pédiculés et libres) ont été utilisés pour corriger des défauts du membre supérieur entre 2014 et 2018. La technique chirurgicale ainsi que les résultats ont été examinés. Les caractéristiques de la peau et la qualité de la cicatrice ont été évalués par l'échelle de Vancouver. La fonction et les symptômes subjectifs ont été évalués par l'échelle DASH (Disabilities of the Arm, Shoulder and Hand).Après un suivi moyen de 39 mois, aucun cas de nécrose du lambeau n'a été observé et aucun problème vasculaire à la main ou d'intolérance au froid n'a été rapporté.Le lambeau antébrachial radial à forme modifiée n'est pas une technique nouvelle, cependant elle est peu connue parmi les chirurgiens de la main. Notre expérience confirme sa fiabilité et montre des résultats esthétiques et fonctionnels acceptables auprès de patients bien sélectionnés.
