Abstract
Delier is een veelvoorkomende complicatie bij oudere patiënten die geopereerd worden aan een abdominaal aneurysma van de aorta (AAA).Prehabilitatie kan de delierincidentie verminderen, maar onderzoek naar het effect hiervan bij AAA-patiënten staat in de kinderschoenen.Ons doel is om de associatie tussen prehabilitatie en delierincidentie te onderzoeken binnen electieve AAA chirurgie. MethodeHet betreft een single-center voor-en-na-studie voor patiënten van ≥ 70 jaar die een electieve AAA-operatie ondergingen, tussen 2013 -2015 (controlegroep) en 2016 -2022 (prehabilitatiegroep).Het programma omvat beoordeling van het delierrisico, fysiotherapie, diëtetiek, ijzerinfusie bij bloedarmoede en een uitgebreide geriatrische beoordeling bij kwetsbaarheid. ResultatenEr zijn 81 controle-en 123 prehabilitatie-patiënten geïncludeerd.De incidentie van delier is na introductie van prehabilitatie verlaagd van 11,1% naar 4,9% (p=.09).Gecorrigeerde regressieanalyse toonde een verlaagde delierincidentie met een aOR van 0.33 (95% CI: 0.1 -1.09, p=.07).Deze analyse is ook uitgevoerd voor de subgroepen open herstel en Endovasculaire Aneurysma Repair (EVAR).De reductie van delier was sterker in de open prehabilitatiegroep (aOR = 0,28, 95% CI: 0,07-1,09, p=0,07) dan in de EVAR prehabilitatiegroep (aOR= 0,80, 95% CI: 0,05 -13,9 , p=.88).Opnameduur was significant korter in de totale prehabilitatiegroep (5 versus 4 dagen, p=.02) en EVAR prehabilitatiegroep (4 versus 3 dagen, p=.001).Ordinale logistische regressie van een gecombineerde uitkomst van de delierduur (in dagen) en 30-dagen mortaliteit (als ergste uitkomst, gescoord als 30 dagen) toonde een mediane delierduur van 2.0 in de controlegroep versus 1.1 in de prehabilitatiegroep (p=.21), waarvan de hoogste 4.1 was bij patiënten met open herstel in de controlegroep. ConclusiePrehabilitatie kan effectief zijn bij het voorkomen van delier bij oudere patiënten die een operatie voor AAA ondergaan, vooral bij patiënten met open herstel.Verder onderzoek is nodig om patiënten te identificeren en te selecteren die het meeste baat hebben bij prehabilitatie.
