Abstract
Apresentação do Caso Apresentamos o caso de um paciente do sexo masculino, de 68 anos, tabagista, com doença de Crohn havia 1 ano, Montreal A3L3B3p. A região perineal apresentava deformidade parcial, fístulas e subestenose do canal anal. Realizou-se um exame proctológico sob anestesia, colocação dos sedenhos, e iniciou-se o tratamento com infliximabe 5 mg/Kg, com resposta satisfatória em 3 meses. Foram retirados os sedenhos. No retorno, o paciente referiu lesões eritemato-descamativas nos cotovelos e no abdome com evolução havia dois meses. O diagnóstico de psoríase foi confirmado por biópsia. Em conjunto com a equipe de dermatologia, optou-se pela suspensão e troca por adalimumabe 40 mg/semana devido ao diagnóstico de hidradenite supurativa concomitante – estágio II de Hurley. O paciente retornou com dor perianal, agravo da subestenose do canal anal, saída de secreção purulenta de múltiplos orifícios fistulosos, e piora das lesões psoriasiformes no dorso, no abdome, nos membros inferiores e superiores, inguinal e periorbital. Optou-se por internação, drenagem dos abscessos perianais e colocação de sedenhos. Foram iniciados corticoide e antibioticoterapia, e fez-se uma solicitação de ustequinumabe.
