SARCOMA DE PARTES MOLES EM PACIENTE COM DOENÇA DE CROHN EM USO DE TERAPIA IMUNOSSUPRESSORA: RELATO DE CASO

Abstract
Apresentação do Caso Paciente do sexo masculino, de 41 anos, com doença de Crohn (DC), havia 8 anos (Montreal A2L3B2), que havia sido submetido previamente a ileocolectomia direita por videolaparoscopia, sem intercorrências. Ele estava em uso de azatioprina (AZA, 2 mg/Kg/dia) para a prevenção da recidiva pós-operatória, em virtude dos fatores de mau prognóstico (cirurgia prévia e idade jovem). Entretanto, após quatro meses de tratamento, o paciente apresentou diarreia, emagrecimento e necessidade de esteroides. O estadiamento revelou recorrência endoscópica (Rutgeerts i2B). Optou-se pela associação com infliximabe (IFX), e o paciente apresentou remissão clínica e endoscópica. Após dois anos de tratamento, optou-se pela manutenção do IFX, em monoterapia. Em 2019, o paciente apresentou perda de resposta ao tratamento, e decidiu-se pela reintrodução da AZA e aumento da dose do IFX. Novamente, houve resposta completa ao tratamento. Em novembro de 2021, o paciente notou crescimento de um nódulo endurecido na coxa direita, indolor. Em janeiro de 2022, passou em consulta com o ortopedista, que indicou excisão local do nódulo tumoral. O anatomopatológico diagnosticou um mioepitelioma maligno (cT1 cN0 cM0). Optou-se pela suspensão do IFX e da AZA, sem sinais de recorrência inflamatória e neoplásica.
