Abstract
A 13-month-old male presented to the Emergency Department with bilateral leg bruising and refusal to stand that had progressed over a 2-month period. There was associated fatigue, pallor, and estimated 1 kg weight loss over this time. He demonstrated oral aversion to solids and was almost exclusively breastfed. This was thought to be secondary to a mild caustic injury due to toilet bowl cleaner ingestion at 8 months of age. Family history was unremarkable. Physical exam revealed tachycardia, mild hepatosplenomegaly, and an enlarged posterior auricular lymph node. Blood work 1-month prior demonstrated normocytic anemia (Hb 98 g/L, mean corpuscular volume 88 fL). Repeat blood work on presentation revealed progression to pancytopenia, with macrocytic anemia (Hb 61 g/L, MCV 100 fL), mild lymphopenia (3.7 10*9/L), and thrombocytopenia (116 10*9/L). Blasts were noted on peripheral smear (absolute 0.4, <5%). Lactate dehydrogenase was elevated (2,171 U/L) and Epstein-Barr virus and cytomegalovirus serologies were negative.
