Ansätze zur Volumenbestimmung von Meningeomen in der PET/CT Bildgebung mit dem neuartigen Tracer F-18-SiTATE

Abstract
Ziel/Aim F-18-SiTATE ist ein neuartiges F-18-markiertes, an Somatostatinrezeptoren bindendes Peptid. Dieses etablierte sich bisher in der Bildgebung von NET. Da oftmals ein alleiniges MRT die ossäre Infiltration eines Meningeoms nicht beurteilen kann, kann das F-18-SiTATE PET/CT (bisher: Ga-68 markierte Somatostatin Analoga) aufgrund der Bindungseigenschaften hierfür genutzt werden. Ziel war die Etablierung einer standardisierte Methode zur Volumenbestimmung von Meningeomen, die bis dato nicht existiert, jedoch für die Planung von OP und Radiatio von großer Relevanz ist. Methodik/Methods Es wurden im CT gut abgrenzbare Meningeome (Volumen CT ≥1ml) ohne intraossäre Infiltration eingeschlossen. Nach Durchführung des SiTATE PET/CTs wurden die Volumina im CT (mit KM) bestimmt und dienten als volumetrische Referenz. Folgende Ansätze zur Volumenbestimmung im PET wurden verwendet und mit der CT korreliert: fester SUV-Wert, Isokontur Schwellenwertbildung abhängig vom SUV max (SUV%) und Schwellenwerte in Abhängigkeit von Knochenmark (SUV BM ), Parotis (SUV parotis ) und Hypophyse (SUV sella ). Ergebnisse/Results Es wurden 19 Meningeome eingeschlossen. Ein SUV-Wert von 4.0 ergab eine hohe Korrelation mit der Referenz (r=0.783, p<0.0001) und geringe Streuung im Bland-Altman-Plot. Die Korrelation durch Isokontur- sowie Referenzgewebe-basierte Schwellenwertbildung war niedriger oder die Streuung im Bland-Altman-Plot bei großen Läsionen größer. Schlussfolgerungen/Conclusions Die Verwendung eines SUV-Werts von 4.0 eignet sich aufgrund hoher Korrelation, Läsionsgrößen-unabhängiger geringer Streuung und nutzerfreundlicher Anwendung zur Volumenbestimmung von Meningeomen im SiTATE PET/CT. Diese kann in der klinischen Routine ohne spezielle technische Voraussetzungen und unabhängig von variabler SiTATE-Avidität in Referenzgeweben erfolgen.
