Zusammenhang von Übergewicht und metabolischem Syndrom mit Polyneuropathie bei Typ-2-Diabetes und normaler Glukosetoleranz

Abstract
Fragestellung Es gibt vermehrt Hinweise auf einen Einfluss des metabolischen Syndroms (MetS) auf die Entwicklung einer distalen sensomotorischen Polyneuropathie (DSPN). Wir untersuchten, inwiefern Komponenten des MetS sowohl bei normaler Glukosetoleranz (NGT) als auch bei kürzlich diagnostiziertem Typ-2-Diabetes (T2D) mit Parametern der DSPN zusammenhängen. Methodik 180 Teilnehmende mit NGT und 355 mit T2D (Diabetesdauer ≤1 Jahr) der Baseline-Kohorte der Deutschen Diabetes-Studie (NGT/T2D [Mittelwert±SD]: Alter: 43,9±14,0/45,1±7,7 Jahre; Geschlecht: 61/65% männlich; BMI: 26,8±5,0/32,3±6,5 kg/m 2 ; HbA1c: 5,2±0,3/6,5±1,0%) wurden elektrophysiologisch und quantitativ-sensorisch untersucht. Bei 218 Personen (NGT/T2D: n=117/101) wurde zusätzlich die intraepidermale Nervenfaserdichte (IENFD) bestimmt. Nach 5 Jahren wurde eine Subgruppe (NGT/T2D: n=48/53) erneut untersucht. MetS wurde nach International Diabetes Federation (2006) definiert, DSPN nach Toronto Consensus Criteria (2011). Ergebnisse Adjustiert für Alter, Geschlecht, Größe, Rauchen und HbA1c war IENFD invers mit Gewicht (β=-0,306), Anzahl an MetS-Komponenten (AMetSK; β=-0,222) und Vorhandensein abdomineller Fettleibigkeit (AbdAd; β=-0,292) assoziiert, während eine Assoziation zwischen höherer malleolärer Vibrationswahrnehmungsschwelle und Gewicht (β=0,219) bei NGT (P≤0,05), nicht jedoch bei T2D beobachtet wurde. Motorische Nervenleitgeschwindigkeit (N.medianus) und sensibles Nervenaktionspotenzial (N.suralis) waren bei T2D invers mit Gewicht (β=-0,182/-0,286), AMetSK (β=-0,127/-0,128), AbdAd (β=-0,163/-0,165) und MetS (β=-0,169/-0,203) assoziiert (P≤0,05). Höheres Gewicht war bei NGT prädiktiv für eine nach 5 Jahren vorliegende DSPN (β=0,652, P=0,001), während zunehmende AMetSK und Vorliegen eines MetS prädiktiv für eine abnehmende IENFD bei T2D (β=-0,533/-0,388, P=0,001/0,013) waren. Schlussfolgerungen Höheres Gewicht scheint bei NGT mit einer höheren Wahrscheinlichkeit für DSPN nach 5 Jahren einherzugehen, während MetS und dessen Komponenten bei kürzlich diagnostiziertem, gut eingestelltem T2D gegenüber NGT unterschiedlich mit DSPN zusammenhängen.
