ResumoEsta revisão abrange o fator aloimune no aborto espontâneo recorrente (AER).Baseia-se em busca da literatura especializada nos últimos 10 anos (1997)(1998)(1999)(2000)(2001)(2002)(2003)(2004)(2005)(2006), nas bases de dados OVID -Medline, Science Direct e Lilacs, no Pubmed, na Bireme e Google.Resultados: AER é a perda de três ou mais gestações clínicas consecutivas, antes da 20ª semana de gravidez ou com produto fetal com menos de 500g.Pode afetar até 5% dos casais férteis.Cerca de 50-60% dos abortamentos espontâneos tem causas genéticas, mas os AER apresentam baixa incidência destas alterações.Aos AER tem sido creditada etiologia idiopática, de natureza auto ou aloimune em 50 a 60% dos casos.Os mecanismos envolvidos incluem a presença de anticorpos citotóxicos, ausência de anticorpos bloqueadores maternos, compartilhamento de antigenos HLA, hipertaividade das citocinas Th-1 e alterações na função e distribuição das células NK.Aloimunidade se refere às diferenças imunológicas entre indivíduos da mesma espécie.A hipótese é de que a diferença antigênica materno-embrionária seria benéfica para o desenvolvimento do embrião.A histocompatibilidade materno-embrionária pode levar a defeitos na interação mãeembrião, alterando a produção de fatores humorais e celulares maternos, vitais para o sucesso da gravidez.Conclusões: AER constitui 5% das ocorrências obstétricas.Embora a maioria dos estudos recentes mostrem resultados encorajadores da imunoterapia, persistem controvérsias.A imunoterapia é uma opção terapêutica de bons resultados se seguida de investigação e protocolo adequados para o tratamento.Permanece a necessidade de estudos multicêntricos bem desenhados para maiores esclarecimentos.
