Abstract
Hintergrund Unterschiede in Integrität der grauen und weißen Substanz bei asymmetrischem Hörverlust (AHL) und normalem Hören (NH) wurde mit Hilfe von strukturellem MRI und Diffusions-Tensor-Imaging (DTI) untersucht. Methode Neun rechtshändigen Erwachsenen mit AHL links und zwölf gematchte NH erhielten ein 3,0 T-MRI des Gehirns. Von CAT-12 abgeleitete Gewebewahrscheinlichkeitswerte der grauen Substanz (GM) im temporalen, okzipitalen und postzentralen Kortex wurden gemessen. Die Mikrostruktur wurde anhand der von DTI-abgeleiteten fraktionellen Anisotropie (FA) in der temporalen, okzipitalen und postzentralen weißen Substanz bewertet. Für den Gruppenvergleich wurde der Asymmetrieindex (AI) berechnet: ipsi- minus kontralateral geteilt durch ihren Mittelwert. Ein negativer AI spiegelt höhere Werte in der ipsilateralen Region und ein positiver AI höhere Werte in der kontralateralen Region wider. Ergebnisse AHL zeigten ein signifikant höherer FA-AI in der mittleren Temporalregion (Mittelwert -0,03 vs. -0,07; p=0,02). Bei AHL war die GM-AI des inferioren temporalen Kortex (Mittelwert 0,07 vs. 0,04; p=0,02) und des superioren okzipitalen Kortex (Mittelwert 0,0006 vs. -0,07; p=0,02) signifikant höher. Ein tendenziell höherer GM-AI des mittleren temporalen Kortex wurde bei AHL beobachtet (Mittelwert 0,07 vs. 0,05; p=0,08). Schlussfolgerung Unsere Ergebnisse deuten darauf hin, dass AHL links zu einer Reduktion der FA-AI der rechten mittleren Temporalregion führt, was auf eine erhöhte Konnektivität kontralateral zum stärker hörgeminderten Ohr hinweist. Dies könnte auf crossmodale Plastizität zurückzuführen sein, da sich bei AHL ein Verlust der Asymmetrie im oberen Okzipitallappen zeigt, während die GM-Werte des rechten inferioren Temporalkortex bei AHL relativ erhöhte sind.
