Abstract
A previously healthy, unvaccinated 5-month-old male presented to a rural emergency department with a 2-month history of progressive constipation, cough, decreased oral intake, and poor weight gain. His physical examination revealed an alert baby with a sunken fontanelle, a distended abdomen with no organomegaly, and a weight decrease from the 85th percentile to the 10th percentile over 2 months. He was not on any medications or supplements, but he had been on Vitamin D previously which his mother administered for his cough. Pregnancy, delivery, and family history were unremarkable. Given the cough, the patient was sent home with 10 days of amoxicillin and 10 days of prednisolone. Though the cough resolved, the other symptoms did not, leading to his re-presentation and further investigations. Bloodwork revealed a normal complete blood count, creatinine, glucose, urea, C-reactive protein, TSH, cortisol, and albumin levels. Electrolyte panel reported a normal sodium and potassium, but an elevated total of 3.97 mmol/L (ref 2.20 to 2.80 mmol/L), ionized calcium of 2.11 mmol/L (ref 1.09 to 1.25 mmol/L), a low phosphate of 0.89 mmol/L (ref 1.2 to 2.2 mmol/L) and an appropriately suppressed PTH of 0.4 mmol/L (ref 1.4 to 6.8 mmol/L).
