Contexte : Au Canada, la gonorrhée est la deuxième infection bactérienne sexuellement transmissible la plus répandue.Le Programme de surveillance de la résistance des gonocoques aux antimicrobiens (GASP -Canada), un système de surveillance passive qui surveille la résistance aux antimicrobiens chez Neisseria gonorrhoeae au Canada depuis 1985, est la source de ce résumé des données démographiques, de la résistance aux antimicrobiens et du typage des séquences multiantigènes de N. gonorrhoeae (NG-MAST) des isolats gonococciques collectés au Canada en 2021.Méthodes : Les laboratoires de santé publique provinciaux et territoriaux ont soumis des cultures de N. gonorrhoeae et des données au Laboratoire national de microbiologie de Winnipeg dans le cadre du système de surveillance.La résistance aux antimicrobiens et le type moléculaire de chaque isolat reçu ont été déterminés.Résultats : Au total, 3 439 cultures de N. gonorrhoeae ont été reçues des laboratoires du pays en 2021, soit une augmentation de 9,9 % depuis 2020 (n = 3 130).La diminution de la sensibilité au céfixime a augmenté de manière significative (p < 0,001) en 2021 (1,5 %) par rapport à 2017 (0,6 %).Aucun changement significatif dans la diminution de la sensibilité à la ceftriaxone n'a été détecté entre 2017 et 2021 (0,6 %) (p > 0,001); cependant, un isolat résistant à la ceftriaxone a été identifié.La résistance à l'azithromycine a diminué de manière significative (p < 0,001) en 2021 (7,6 %) par rapport à 2017 (11,7 %); toutefois, on a observé une augmentation significative (p < 0,001) de la proportion de cultures présentant une concentration minimale inhibitrice d'azithromycine d'au moins 1 mg/L (2017 = 22,2 % à 2021 = 28,1 %).En 2021, NG-MAST-19875 (15,3 %) était le type de séquence le plus répandu au Canada; 20,3 % des isolats présentant ce type de séquence étaient résistants à l'azithromycine.Conclusion : La propagation de la gonorrhée résistante aux antimicrobiens est un problème de santé publique important.La poursuite de la surveillance régionale et nationale de la résistance aux antimicrobiens chez N. gonorrhoeae est essentielle pour garantir que des traitements efficaces sont recommandés.
