Pemanfaatan teknologi informasi dalam bidang kesehatan menjadi aspek krusial mengingat kesehatan menjadi fundamental dalam pelaksanaan pembangunan berkelanjutan (Sustainable Development Goals). Salah satu hal yang perlu ditingkatkan adalah penyelenggaraan layanan publik di tingkat pemerintah desa menggunakan konsep smart village pada layanan kesehatan untuk meningkatkan nilai SDGs desa. Metode dalam riset ini menerapkan framework TOGAF ADM 9.2 dimana tersusun atas 7 (tujuh) fase yakni Preliminary, Architecture Vision, Information System Architecture, Business Architecture, Technology Architecture, Opportunities and Solution, dan Migration Planning. Perancangan Enteprise Architecture Smart Village sesuai Peraturan Presiden Republik Indonesia Nomor 111 Tahun 2012 mampu membantu pemerintah desa meningkatkan taraf hidup sekaligus mencapai pembangunan berkelanjutan.
