Abstract

Un homme de 62 ans de race blanche s’est présenté à un examen de routine avec les antécédents d’un AVC de l’artère cérébrale moyenne (ACM) gauche survenu cinq ans auparavant, ce qui a entraîné une aphasie importante, mais aucun déficit sensori-moteur ou visuel initial. Un épisode convulsif a nécessité une hospitalisation deux ans plus tard et de multiples attaques corticales et sous-corticales bilatérales ont été révélées par des tests par IRM subséquents. Au moment où il s’est présenté, les meilleures acuités corrigées étaient de 20/20+ O.U. par correction astigmatique hypermétropique légère. L’évaluation du champ visuel avec la technologie de doublement de fréquence a révélé une quadranopsie supérieure gauche incongrue. Les segments antérieurs présentaient une légère blépharite mixte bilatérale, mais n’étaient autrement pas notables. L’examen du fond d’œil sous pupille dilatée a révélé une légère pâleur du disque optique temporel et une excavation O.U. modérée. La tomographie par cohérence optique (TCO) en domaine spectral a révélé un amincissement marqué et symétrique de la couche inférieure et temporelle des couches de fibres nerveuses rétiniennes (CFNR), ainsi qu’un amincissement bilatéral diffus des cellules ganglionnaires rétiniennes. Ce cas montre comment la détection par TCO de la perte rétrograde des fibres nerveuses et des cellules ganglionnaires peut être plus efficace que le dépistage visuel pour révéler l’étendue des lésions post-chiasmatique dans le contexte de multiples AVC.
