Postoperativer Chemotherapie-Einsatz und Ergebnisse der ADAURA-Studie: Osimertinib als adjuvante Therapie beim resezierten EGFR-mutierten NSCLC

Abstract
Einleitung: Etwa 30% der Patienten (Pat) mit nicht-kleinzelligem Lungenkarzinom (NSCLC) sind operabel. Eine adjuvante Chemotherapie (adjCTx) wird bei Pat mit reseziertem NSCLC im Stadium (St) II–IIIA (selektiv StIB) empfohlen; Rezidive sind dennoch häufig. In der doppelblinden, randomisierten Phase-III-Studie ADAURA (NCT02511106) zeigte Osimertinib (OSI) eine statistisch signifikante und klinisch relevante Verbesserung des krankheitsfreien Überlebens (DFS; Hazard Ratio [HR]: 0,20 [99,12% CI 0,14, 0,30]; p < 0,001) bei Pat mit vollständig reseziertem EGFR-Mutations-positiven (EGFRm) NSCLC im St IB–IIIA nach adjCTx, sofern indiziert. Wir berichten in dieser explorativen Analyse über Einsatz und Einfluss adjCTx auf ADAURA-Studienergebnisse. Methoden: Pat wurden 1 : 1 randomisiert und erhielten 80 mg/Tag OSI oder Placebo (PBO) bis Studienende (3Jahre [J]) oder Rezidiv. Postoperative AdjCTx konnte als Standard vor Randomisierung nach Wahl des Arztes durchgeführt werden, war aber nicht zwingend erforderlich und wurde explorativ ausgewertet. Die DFS-Analyse in der Gesamtpopulation (StIB-IIIA, ± adjCTx) war eine vordefinierte Subgruppenanalyse, die mit einem Cox-Proportional-Hazard-Modell durchgeführt wurde. Daten-Cutoff: 17/01/20. Ergebnisse: 60% (410/682) aller Pat erhielten eine adjCTx, Anteil sinkend mit zunehmendem Alter (66% < 70J [338/509], 42% ≥ 70J [72/173], 27% ≥ 75J [21/78], mediane Dauer 4.0 Zyklen, Behandlungsarme ausgeglichen) und kleinerem Stadium (IB: 26% [57/216], II: 71% [165/231]; IIIA: 80% [187/235]). Der WHO-Performance-Status (PS) beeinflusste den CTx-Einsatz nicht. Die DFS-Analyse zeigt vergleichbare Ergebnisse ± CTx (HR: 0,16 [95% CI 0,10, 0,26] vs. HR: 0,23 [95% CI 0,13, 0,40]), stadienunabhängig in der Gesamtpopulation. Schlussfolgerung: Die Anwendung von adjCTx in der ADAURA-Studie war vergleichbar mit früheren Studien und der klinischen Praxis. Wie erwartet waren jüngeres Alter und höheres Krankheitsstadium mit einem erhöhten CTx-Einsatz assoziiert, jedoch PS-unabhängig. Der Vorteil im DFS OSI vs. PBO war unabhängig von CTx-Einsatz und Stadium.
