A Cross-Sectional Study on the Relationship Between Alexithymia and Psychiatric Symptoms in Patients with Schizophrenia: A Preliminary Report

Abstract
Amaç: Çalışmamızın amacı, aleksitiminin şizofreni hastalarında görülme oranlarının tespit edilmesinin yanı sıra aleksitiminin diğer klinik belirtilerle olan ilişkisini ortaya koymaktır. Bu sayede, konuyla ilgili alanyazına katkı sağlanabileceği düşünülmektedir. Gereç ve Yöntemler: Çalışmaya, Ankara Üniversitesi Tıp Fakültesi Hastanesinin Ruh Sağlığı ve Hastalıkları Ana Bilim Dalı poliklinik birimine Kasım 2021-Nisan 2022 yılları içerisinde ayaktan başvuran, 18-65 yaş arasındaki ve ''The Diagnostic and Statistical Manual of Mental Disorders (DSM-5)'' tanı ölçütlerine göre ''şizofreni'' tanısı konmuş 32 hasta dâhil edilmiştir. Hastalara sosyodemografik veri formu, Toronto Aleksitimi Ölçeği (TAÖ-20) ve DSM-5 Birinci Düzey Kesitsel Belirti Ölçeği (Yetişkin) uygulanmıştır. Bulgular: TAÖ-20'den aldıkları toplam puanların ortalaması 64,34±14,45 olarak bulunurken, aleksitimisi olanlar katılımcıların %59,4'ünü oluşturmaktadır. Aleksitimik olan şizofreni tanılı bireylerde depresyon (p=0,038), tekrarlayan düşünce ve davranışlar (p=0,030) ve kişilik bozukluğu (p=0,010) belirtilerinin daha fazla görülmesinin istatistiksel olarak anlamlı olduğu belirlenmiştir. Depresyon, öfke, tekrarlayan düşünce ve davranışlar ve kişilik bozukluğu belirtilerinin hem aleksitimi toplam puanları (depresyon p=0,003; öfke p=0,017; tekrarlayan düşünce ve davranışlar p=0,019; kişilik bozukluğu p=0,002) hem de aleksitiminin duygu tanımada güçlük (depresyon p=0,002; öfke p=0,008; tekrarlayan düşünce ve davranışlar p=0,007; kişilik bozukluğu p=0,009) ve duygu söze dökmede güçlük (depresyon p=0,000; öfke p=0,021; tekrarlayan düşünce ve davranışlar p=0,006; kişilik bozukluğu p=0,001) alt boyutlarıyla ilişkili bulunmuştur. Sonuç: Verilerimiz, şizofreni tanılı bireylerde aleksitiminin, bazı ruhsal belirtilerle ilişkili olabileceğini göstermektedir.
