Hipurato de Metenamina na Prevenção de Infeções do Trato Urinário de Repetição: Uma Revisão Baseada na Evidência

INTRODUÇÃO: A antibioterapia profilática é utilizada de forma a prevenir a recorrência de infeções do trato urinário (ITUs), contudo a crescente problemática da resistência à antibioterapia ao longo dos últimos anos tem levado à procura de alternativas. O objetivo desta revisão foi avaliar a evidência disponível acerca da eficácia do hipurato de metenamina como alternativa à antibioterapia na profilaxia das ITUs recorrentes.MÉTODOS: As fontes de dados consultadas foram The Cochrane Library, MEDLINE/PubMed, DARE, TRIP Medical Database, Canadian Medical Association Practice Guidelines Infobase e National Guideline Clearinghouse. Foi realizada a pesquisa de meta-análises, ensaios clínicos controlados e aleatorizados, estudos observacionais e guidelines escritos em inglês ou português, publicados entre janeiro de 2012 e maio de 2022, tendo sido utilizados os termos MeSH “methenamine” e “urinary tract infections”. O nível de evidência e a força de recomendação foram avaliados com base na escala Strength of Recommendation Taxonomy (SORT) da American Family Physician.RESULTADOS: Dos 106 resultados obtidos inicialmente, foram selecionados 3 artigos, 2 ensaios clínicos controlados e aleatorizados e 1 estudo observacional retrospetivo. A evidência encontrada nestes estudos revelou que o hipurato de metenamina é equiparável à antibioterapia profilática na prevenção das ITUs recorrentes.CONCLUSÃO: Esta revisão permite concluir que o hipurato de metenamina poderá surgir assim como uma possível alternativa viável e segura à antibioterapia profilática demonstrando não ser inferior a esta (SORT B), tendo a vantagem clara de não contribuir para a problemática crescente que é a resistência antibiótica.
