Abstract
Introduction: Small bowel transplantation is currently used as a therapeutic option in selected cases of permanent pediatric intestinal failure who experience life-threatening complications from parenteral nutrition such as end stage liver disease, lack of venous access and IV line infection. On a worldwide basis two out of every three SBT have been performed in children under 18 years of age. The success rate of SBT is quite low (<40% 5-year survival). A major obstacle to successful small bowel transplantation is that of bacterial infection as a direct result of bacterial translocation across an injured bowel graft related to preservation. The aim of this study was to improve graft quality during cold storage by combining two proven preservation strategies involving perfluorocarbon and a novel luminal preservation solution. Methods: Small bowel from Sprague-Dawley rats (n=30) was flushed vascularly with modified UW solution and flushed luminally as below: Group 1- none (control); Groups 2 and 3 – UW solution; Groups 4 and 5 – an amino-acid rich solution (a mixture of 19 amino acids based on physiological requirements). Groups 2–5 were then submersed in perfluorodecalin (pre-saturated with oxygen). In Groups 3 and 5, perfluorocarbon solution was bubbled continuously with 100% oxygen at a rate of 50 to 100 mL/min. Energetics, oxidative stress and histology were assessed over 24 h at 4oC. Results: ATP levels were consistently greater than control throughout 24 h in Groups 2 to 5. Of particular clinical relevance, Group 4 exhibited significantly greater ATP and ATP/ADP ratios and energy charge level after 12 h storage; values were almost 2 fold greater than values in other Groups. Even after 24 h Group 4 energetic values was significantly higher than those of other groups indicating a greater reserve of high-energy molecules and, thus, better preserved tissue. In terms of lactate accumulation, levels of this end product were elevated in Groups 3 and 5 compared with other groups during the entire 24 h. Overall, histologic injury was lower in amino acid treated tissues than others; after 12 h storage median injury in Groups 4 and 5 indicated villus denudation with only 1/6 specimens exhibiting evidence of crypt infarction. Histologic injury was more prevalent in Groups 1, 2 and 3 with a greater incidence of crypt and transmucosal infarction. Even after 24 h, Groups 4 and 5 showed superior mucosal integrity. Conclusion: Our data indicate that combined treatment with perfluorocarbon and luminal amino acid–rich preservation solution significantly improves energetics and histologic integrity. This combined strategy may have implications for the successful preservation and transplantation of SB in the clinic.
