Praktikabilität des Einsatzes eines T-Piece-Device (Neo-Tee®) zur respiratorischen Erstversorgung von präklinisch geborenen Neugeborenen

Abstract
Fragestellung Innerklinisch wird die Beatmung Neugeborener im Rahmen der Erstversorgung für gewöhnlich mittels sog. T-Piece-Devices durchgeführt. Im prähospitalen Setting werden bei Außengeburten hierfür durch NotärztInnen und SanitäterInnen üblicherweise Beatmungsbeutel eingesetzt, die im Vergleich Nachteile aufweisen. Mit Neo-Tee® gibt es nun preisgünstige Einweg-T-Piece-Devices, die zur Optimierung der Neugeborenen-Versorgung auch Anwendung in der Präklinik finden könnten. Kann mit dieser Beatmungshilfe im Modellversuch eine suffiziente Beatmung Neugeborener erzielt werden?Methodik Es wurde von 25 NotfallsanitäterInnen des Österreichischen Roten Kreuzes in dieser randomisiert-kontrollierten Mannequinstudie eine jeweils einminütige Beatmung eines leckfreien neonatalen Beatmungsmodells mittels Neo-Tee® (Mercury Medical, Clearwater, Florida, U.S.A.) und Beatmungsbeutel (Ambu® Mark IV Baby, Ambu, Ballerup, Dänemark) durchgeführt. Die NotfallsanitäterInnen wurden angewiesen Voreinstellungen am Neo-Tee® zu treffen um bei 8 l/min Gasfluss einen end-exspiratorischer Druck [PEEP] von 5 cmH2O und einen Spitzendruck [PIP] von 25 cmH2O zu erreichen. Beatmungsparameter (PIP, VTe, PEEP, Maskenleckage, Beatmungsfrequenz) wurden mittels Respiratory Function Monitor erfasst.Ergebnisse Es wurden insgesamt 50 Minuten mit rund 2250 Beatmungen aufgezeichnet und analysiert. Mit Neo-Tee® bzw. Beatmungsbeutel zeigte sich ein mittlerer (Standardabweichung) PIP von 22,2 (1,5) bzw. 32,4 (11,5) cmH2O (p < 0,001), ein VTe von 40,8 (13,1) bzw. 29,7 (8,7) ml (p = 0,001), ein PEEP von 2,7 (0,8) bzw. 3,9 (1,5) cmH2O (p = 0,001), eine Maskenleckage von 16 (24) bzw. 42 (25) % (p < 0,001), sowie eine Beatmungsfrequenz von 43 (11) bzw. 39 (10) min-1 (p = 0,032).Interpretation Die Beatmung Neugeborener mittels Neo-Tee® ist im Modellversuch durch NotfallsanitäterInnen in hoher Qualität möglich. Im Vergleich zur Beutel-Masken-Beatmung zeigten sich niedrigere Spitzendrücke und höhere exspiratorische Tidalvolumina bei einer niedrigeren Maskenleckage, was auf eine effektive und sicherere nicht-invasive Beatmung hinweist. Die niedrigeren PEEP-Werte bei der Beatmung mittels Neo-Tee® könnten durch höhere Floweinstellungen kompensiert werden.
