Neoadjuvante chemotherapie of primaire debulking bij het vergevorderde ovariumcarcinoom

Auteur(s): VERGOTE I, VERLEYE L, VAN GORP T, LEUNEN K, NEVEN P, AMANT F | Samenvatting: Het ovariumcarcinoom treedt nog steeds vaak in een vergevorderd stadium op en heeft een sombere prognose. Debulkingchirurgie en op platina gebaseerde chemotherapie vormen de hoekstenen van de behandeling. Bij patiënten met een vergevorderd ovariumcarcinoom werd traditioneel primair een debulkingchirurgie uitgevoerd. Recent werd een nieuwe strategie ontwikkeld waarbij patiënten eerst neoadjuvante chemotherapie krijgen gevolgd door een intervaldebulking.Uit een recente, gerandomiseerde studie van de European Organisation for Research and Treatment of Cancer – National Cancer Institute Canada (EORTC-NCIC) bleek dat bij patiënten met een zeer uitgebreid ovariumcarcinoom in stadium IIIc of IV de overleving even goed is bij neoadjuvante chemotherapie gevolgd door intervaldebulking in vergelijking met het uitvoeren van een primaire debulking. De postoperatieve verwikkelingen en de mortaliteit waren lager na intervaldebulking dan na primaire debulkingchirurgie. Afwezigheid van resttumor was de belangrijkste onafhankelijke prognostische factor zowel bij primaire als bij intervaldebulkingchirurgie.Bij sommige patiënten is de chirurgie na neoadjuvante chemotherapie niet eenvoudig door de ontwikkeling van chemotherapiegeïnduceerde fibrose. Verder is het belangrijk te benadrukken dat de gerandomiseerde patiënten een zeer uitgebreid ovariumcarcinoom in stadium IIIc of IV hadden, en dat bij patiënten met metastasen kleiner dan 5 cm de overleving iets beter was na primaire debulking. Daarom is een goede selectie van patiënten met een ovariumcarcinoom in stadium IIIc of IV voor primaire chirurgie of intervalchirurgie van essentieel belang. Goed beeldvormend onderzoek (door middel van CT met of zonder PET) en laparoscopie spelen hierin een belangrijke rol.
