Abstract
Résumé La ventilation non invasive (VNI) est devenue le support ventilatoire de première intention en service de réanimation pédiatrique. Le succès de la technique repose sur des interfaces adaptées à la morphologie de l’enfant et la gestion des fuites, ce qui représentent une charge de travail paramédicale (et médicale) importante mais indispensable. Le choix de l’interface, masque facial ou interfaces classiques (nasales notamment) est essentiel pour diminuer les fuites et améliorer la synchronisation particulièrement chez le nourrisson. Les modes barométriques sont les plus fréquents que les modes volumétriques avec le réglage de 1 (CPAP) ou 2 niveaux de pression (BIPAP). Les mesures associées (alimentation, soins, confort) ne doivent pas être oubliées. Aujourd’hui la VNI est validée dans de nombreuses pathologies aiguës (bronchiolite, pneumopathie, exacerbation sévère d’asthme, syndrome thoracique aigu…) et même dans les formes modérées de syndrome de détresse respiratoire aiguë à condition de ne pas retarder l’intubation chez les patients non répondeur à la ventilation non invasive. La réévaluation doit donc être précoce (dès les 2 premières heures) pour adapter le support et identifier les patients nécessitant une ventilation invasive. Ainsi, la VNI ne s’improvise pas et nécessite un environnement favorable tant sur le plan matériel que sur les compétences médicales et paramédicales.
