Abstract
Sehr geehrte Herausgeber, ein 69-jähriger Patient, mit einem Myokardinfarkt nach Angioplastie, arterieller Hypertonie und chronischer Niereninsuffizienz in der Anamnese, stellte sich mit einem, seit fünf Tagen bestehenden progredienten Erythem vor, das am Thorax begann, sich dann generalisierte und den größten Teil der Hautoberfläche erfasste.Außerdem klagte er seit zwei Tagen über Schwellungen im Gesicht, Pusteln und großflächigen Hautablösungen sowie eine zunehmende Empfindlichkeit der Haut.Darüber hinaus hatte er leichte Schmerzen im Mund und berichtete über Müdigkeit und ein Unwohlsein.Fünfzehn Tage vor Beginn der Symptome verschrieb ihm sein Hausarzt eine dreiwöchige Behandlung mit Terbinafin 250 mg pro Tag gegen einen juckenden Hautausschlag in der Leistenregion.Drei Tage vor Beginn der Symptome wurde ihm in der Notaufnahme eine fünftägige Behandlung mit Prednison 50 mg täglich wegen eines synkopalen Anfalls verordnet.Zu seinen weiteren Langzeitmedikationen gehörten Ezetimib, Clopidogrel, Aspirin und Metoprolol.Darüber hinaus berichtete er über seit den letzten vier Jahren bestehende chronische erythematöse, dicke schuppende Plaques an den Ellenbogen.Die Untersuchung ergab einen ausgedehnten Hautausschlag (Abbildung 1a-c), und es wurden Hautablösungen (Abbildung 1d, e) festgestellt, die sich über 30%
