Aplicação do “feixe de intervenções” - prevenção de infeção urinária associada ao cateter vesical numa unidade intermédia polivalente

Abstract
Introdução: A Organização Mundial da Saúde distingue a infeção urinária associada ao cateter vesical como uma importante causa de eventos adversos. Programas de auditoria são fundamentais e permitem avaliações sistemáticas e definição de estratégias educativas estruturadas. Objetivos: Avaliar o grau de conformidade entre a prática clínica e o cumprimento do feixe de intervenções de prevenção de infeção urinária associada ao cateter vesical. Metodologia: Estudo descritivo, transversal, realizado numa Unidade Intermédia Polivalente do norte de Portugal, com uma amostra constituída por 20 enfermeiros e 10 assistentes operacionais. A recolha de dados teve por base a consulta de processos clínicos e a observação direta da prática clínica com recurso a uma grelha de auditoria baseada na norma “Feixe de intervenções de prevenção de infeção urinária associada a cateter vesical” da Direção Geral da Saúde e no documento da American Nurses Association – “Streamlined Evidence – Based RN Tool: Catheter Urinary Tract Infection Prevention”. Resultados e Discussão: A maioria das intervenções alcançou índices de conformidade superiores a 90%, mas a conformidade geral foi penalizada, devido à baixa adesão da equipa à fixação adequada do cateter, à higiene das mãos antes e após sua manipulação, à higiene diária do meato uretral e ao esvaziamento e manipulação do saco coletor. Conclusão: Intervenções bem elaboradas, como as desenvolvidas nos bundles, são fundamentais para reduzir o número de infeções associadas aos cuidados de saúde. A sua implementação exige conhecimento de toda a equipa, a adesão às medidas preventivas, a competência técnica e treinos periódicos.
