Pooled Cord Blood Mesenchymal Stromal Cells Slow the Progression of Osteoarthritis in an Equine Fetlock Chip Model

Abstract

Introduction: We hypothesize that equine pooled cryopreserved umbilical cord blood (eCB) MSCs are safe and effective in treating joint pain and inflammation in an equine model compared with placebo. Materials and Methods: Osteoarthritis (OA) was surgically induced in 12 Standardbreds using a fetlock chip model. Horses were exercised throughout the study. Horses were randomly assigned to be treated with either 10 million pooled cryopreserved eCB- MSCs in HA ( n = 6) or NaCl ( n = 6). At various time points pre and postoperatively, and post treatment, lameness evaluations, radiographs, ultrasound, MRI and synovial fluid samples were taken, scored and statistically analyzed. Results: There was a significant decrease in the AAEP lameness scores over time for the MSC group [p:0.03]. Lameness locator supported these results (mean vector sum differences MSC 18.58 [15.6–22.1], control 28.6 [24.1–34.1], at week 11 [p:0.009]). Synovial fluid total nucleated cell count was overall significantly higher in the MSC group compared with the control [ p = 0.02]. The median radiographic OA score pre surgery was 0/39 for both groups [MSC 0, control 0–9]. By week 11, it was 3/39 [0–7] in the MSC group, compared with 9/39 [0–13] in the control. The mean difference WORMS score was significantly lower in the MSC group, -0.83 [SD 0.70] (p:0.046), compared with 2.83 [SD 1.77] in the control. The mean difference between arthroscopic evaluations was not statistically significant ( p = 0.07), 11.83 [SD 0.6] in the MSC group and 16 [SD 2] in the control. Discussion/Conclusion: Our study confirms the safety and effectiveness of eCB MSCs in slowing OA progression in a fetlock chip model. Acknowledgment: CB MSC were provided by eQcell in kind.
