Comparable Results with 10 Million or 20 Million Allogenic Mesenchymal Stromal Cells in Treating Synovitis in Horses

Abstract
Introduction: This prospective blinded randomized clinical study aimed to evaluate the safety and effectiveness of a single intraarticular administration of either 10 or 20 million CB - MSCs in HA for treating naturally occurring synovitis in client-owned horses. Materials and Methods: Horses were included if blocking localized a lameness to the fetlock or carpus. Horses were randomly assigned to intra-articular treatment with either 10 or 20 million allogenic CB-MSC’s in HA. Horses were evaluated for effusion (0–4) and lameness (0–5) prior to treatment, and at 72 hours, 3- and 6-weeks post-treatment. Results: Of 18 horses total, 9 horses each were randomly allocated to 10 or 20 million CB-MSC group. In the 10 million MSC group, the median (range) of lameness at the time of treatment was ⅗ (2–3.5) and improved to ⅕ (0–3) at 3 and 6 weeks. In the 20 million MSC group the median (range) of lameness at the time of treatment was ⅖ (2–3) and improved to ⅕ (0–2) at 3 and to ⅕ (range 0–1) at 6 weeks. Overall, the lameness improved significantly in both groups by at least 1 degree compared with the lameness present before MSC treatment ( p = 0.0001), without difference between groups ( p = 0.29). ⅞ (87%) from the 10 million, and 6/8 (75%) from the 20 million MSC group returned to the same level of exercise. Discussion/Conclusion: 10 or 20 million CB MSC were equally effective in reducing lameness in naturally occurring synovitis in client owned horses.
