Abstract
Osteomielitis merupakan inflamasi tulang akut maupun kronis pada pasien dengan fraktur terbuka dan (walaupun jarang) fraktur tertutup. Osteomielitis dapat terjadi pada semua usia dengan angka kejadian lebih tinggi pada anak dan lebih sering terjadi pada laki-laki. Hingga saat ini belum ada angka pasti tentang insidensi osteomielitis di Indonesia. Osteomielitis dapat menyebar secara hematogen dan inokulasi langsung atau penyebaran langsung infeksi ke tulang. Tatalaksana osteomielitis memiliki tantangan tersendiri karena perlu mempertimbangkan patogen penyebab infeksi yang pada beberapa kasus disebabkan oleh patogen yang tidak umum menyebabkan osteomielitis. Penderita diabetes seringkali akan mengalami diabetic foot infection (DFI), terutama pada kadar gula yang tidak terkontrol. Diabetic foot osteomyelitis (DFO) merupakan konsekuensi paling umum penyebaran infeksi jaringan lunak ke tulang pada DFI. Keterlibatan tulang harus dicurigai pada seluruh pasien diabetes dengan ulkus terutama pada ulkus kronik dan rekuren serta temuan klinis yang mendukung adanya infeksi. Kami melaporkan pasien laki-laki dengan riwayat diabetes mellitus berusia 38 tahun dengan osteomielitis kronis yang terjadi pada fraktur tertutup pada tulang calcaneus. Pasien memiliki riwayat trauma. Hasil pemeriksaan kultur dan sensitivitas antiobiotik didapatkan patogen infeksi Acinetobacter baumannii yang sensitif terhadap Amikacin. Debridement, sequestrectomy dan pemberian bone cement serta pemberian antibiotik empirik Amikacin dilakukan pada pasien ini. Hasil evaluasi post pembedahan pasien baik. Penegakkan diagnosis yang cepat dan akurat dengan mempertimbangkan temuan klinis, riwayat penyakit atau komorbid dan temuan laboratoris, serta tatalaksana yang tepat seperti debridement dan intervensi pembedahan dapat memberikan hasil dan prognosis yang baik pada osteomielitis kronis.
