Abstract
Background : Stroke is the most debilitating and deadly neurovascular disease. 80% of strokes are ischaemic, with atheroma of the aortic arch and supra-aortic trunks being the aetiology in 20% to 40%. Its aetiological assessment, for a long time based on Doppler ultrasound and conventional angiography, is now based on MRI and multimodality CT-scan. Objective : Describe the etiological lesions of ischemic strokes and their impact on the brain parenchyma using CT angiography of the supra-aortic trunks. Material and methods : This was a cross-sectional study carried out at the medical imaging service of Montluçon Hospital (France), over a period of 6 months from December 2018 to May 2019. Its included all patients who undergo CT angiography of the supra-aortic trunks for a symptomatology suggesting a cerebral vascular attack. The studied variables were: age, sex, ct-scan indications and scan results (vascular and parenchyma lesions). Statistical analysis was performed using the software SPSS 20 with the Chi-square and Fisher tests at the 5% significance level (P<0,05). Results : The mean age was 65.87 (±20.92) with a male predominance and a sex ratio of 1.25. Cerebral parenchymal lesions represented 16.61% with a predominance of ischemic stroke in 88% of cases. Vascular lesions were mainly found on the supra-aortic trunks in 87.06% of cases. On the supra-aortic trunks, atheroma lesions represented 90.5% and located preferentially on the internal carotid artery in 62.5% of cases. Stroke parenchymal lesions were significantly associated with of the supra-aortic trunks involvement at the 5% level (p=0.000217). Conclusion : CT angiography of the supra-aortic trunks is a main exploration in the etiological assessment of cerebrovascular accidents, particularly for ischemic stroke. RÉSUMÉ Introduction : L’AVC constitue la pathologie la plus invalidante et la plus mortelle des affections neuro-vasculaires. Quatre-vingt pourcent (80%) des AVC sont ischémiques avec pour étiologie l'athérome de la crosse aortique et des troncs supra-aortiques dans 20 % à 40 %. Son bilan étiologique, longtemps basé sur l’écho doppler et l’angiographie conventionnelle repose de nos jours sur l’IRM et le scanner multimodal. Objectif : Décrire à l’angioscanner des troncs supra-aortiques les lésions étiologiques des AVC ischémiques et leur retentissement dans le parenchyme cérébral. Matériel et méthodes : Il s’agissait d’une étude transversale descriptive réalisée au service d’imagerie médicale du Centre Hospitalier de Montluçon (France), sur une période de 6 mois allant de Décembre 2018 à Mai 2019. Ont été inclus dans l’étude tous les patients ayant réalisé un angioscanner des troncs supra-aortiques (TSA) pour une symptomatologie évoquant une atteinte vasculaire cérébrale. Les variables étudiées étaient : l’âge, le sexe, les motifs d’examen et les données scanographiques. L’analyse statistique des données a été réalisée par le logiciel SPSS 20 avec les tests de Chi2 et Fisher au seuil de significativité fixé à 5% (P<0,05). Résultats : La moyenne d’âge était de 65,87 (±20,92) ans avec une prédominance masculine soit un sex-ratio de 1,25. Les atteintes parenchymateuses cérébrales représentaient 16,61% avec une prédominance des AVC ischémiques dans 88% des cas. Les lésions vasculaires étaient principalement retrouvées sur les TSA dans 87,06% des cas. Sur les TSA, les lésions vasculaires athéromateuses représentaient 90,5% et siégeaient préférentiellement sur la carotide interne dans 62,5% des cas. Les lésions parenchymateuses d’AVC étaient significativement liées à une atteinte sur les TSA au seuil de 5% (p=0,000217). Conclusion :L’angioscanner des TSA constitue une stratégie d’exploration étiologique à large spectre de la pathologie vasculaire cérébrale, en particulier de l’AVC ischémique.
