Abstract
A Técnica de Intubação Rápida (TIR) foi desenvolvida com o intuito de prevenir a aspiração do conteúdo do estômago, por meio da pressão na cartilagem cricóida e da utilização de barbitúricos e succinilcolina para indução, um problema comum durante a intubação de emergência. Atualmente, a abordagem da TIR na emergência é bastante diferente do que costumava ser no início. No entanto, seus objetivos principais ainda são os mesmos, focando principalmente em garantir uma via aérea desobstruída e prevenir a aspiração do conteúdo gástrico. Considerando esses pontos, é importante analisar os avanços constantes na TIR, desde o preparo e pré-oxigenação até a indução da paralisia e o manejo do paciente durante a intubação, passagem do tubo e após o procedimento, destacando os principais avanços e discussões em cada uma dessas etapas. A TIR é indicada para pacientes com insuficiência respiratória aguda, seja hipoxêmica (por distúrbios nas trocas gasosas) ou hiperneumónica (por falência ventilatória), bem como para pacientes que, devido a alterações no estado mental, não conseguem proteger suas próprias vias aéreas. A técnica de TIR requer precauções e preparo adequados para reduzir complicações e garantir sua eficácia. Além do preparo, é fundamental monitorar o paciente durante todo o procedimento de intubação.
