ANTECEDENTE: Los procedimientos quirúrgicos más utilizados para el tratamiento CIN2/3+ por VPH son las conizaciones, entre ellas el cono LEEP convencional y el cono largo LLETZ. A pesar de su eficacia y uso común, se ha demostrado recurrencia del virus posterior al tratamiento. OBJETIVO: Evaluar la incidencia de Virus del Papiloma Humano posterior al tratamiento quirúrgico por conización con asa electro quirúrgica (Cono LEEP) y conización con asa larga electro quirúrgico (LLET-Z) en pacientes con displasia cervical de alto grado MÉTODOS: Realizamos una revisión sistemática y un metaanálisis de estudios de Cohorte únicamente, que informaron la recurrencia del Virus del Papiloma Humano, en un periodo de seguimiento mayor de seis meses. La calidad de los estudios se evaluó mediante la escala de Newcastle-Ottawa (NOS). Así como también mediante la lista para estudios de cohortes para evaluar prevalencia del Instituto de Joanna Briggs (JBI). La búsqueda de los estudios de realizó en OVID Medline, PubMed, Web of Science y Scopus hasta el 18 de agosto del 2022. RESULTADOS: Se analizó un total de 11 estudios (n = 4165 mujeres) que indicaran la incidencia del virus del papiloma humano en un tiempo de seguimiento mayor a seis meses. La edad media de las mujeres oscilo entre los 15 a 81 años. Todas las investigaciones incluidas fueron de tipo Cohortes. La mayoría de los estudios utilizo como prueba diagnóstica para recurrencia, un seguimiento por colposcopia, considerando no recurrencia, tres pruebas negativas consecutivas. La recurrencia del virus del papiloma humano posterior a LEEP fue de 2.87% posterior a LEEP (IC 95%/= 2.25 a 3.50) y de 5.42% posterior al procedimiento quirúrgico cono LLETZ (IC 95%/= 4.25 a 6.59). CONCLUSIONES: La recurrencia del virus del papiloma humano posterior a los tratamientos quirúrgicos LEEP y LLETZ, es baja y moderada, respectivamente. Por lo tanto, concluimos que la reinfección se puede presentar en un porcentaje bajo de pacientes a pesar de haber tenido un resultado postquirúrgico satisfactorio.
