Nowotwory głowy i szyi wywołują u pacjentów silny ból, który obniża jakość życia i utrudnia wykonywanie podstawowych funkcji życiowych.Leczenie tego bólu dostępnymi metodami stanowi duże wyzwanie terapeutyczne.Obecnie próbuje się znaleźć rozwiązanie w bezpośrednim blokowaniu mechanizmów odpowiedzialnych za ból.Dokładne mechanizmy molekularne powstawania tego zjawiska nie są jednak do końca poznane.Wśród czynników wywołujących ból w chorobie nowotworowej głowy i szyi istotną rolę odgrywają cytokiny.Są to czynnik martwicy nowotworów α, interleukiny, endoteliny i czynniki neurotroficzne, takie jak czynnik wzrostu nerwów.Wydzielane przez guz i tkankę aktywują i uczulają receptory na neuronach aferentnych.Receptory dla tych cząsteczek to receptory z rodziny TRP oraz ich swoiste receptory.Do powstania bólu przyczyniają się także elementy mikrośrodowiska guza, wśród których znajdują się między innymi proteazy i jony wodorowe.Aktywują i regulują inne receptory bólu -receptory z rodziny PAR.Niniejszy artykuł przedstawia aktualny stan wiedzy dotyczący najpowszechniej badanych patomechanizmów powstawania bólu w nowotworach głowy i szyi.
